Patient Information

All patients are required to provide contact details; including address, telephone and email in order to secure an appointment.

We ask that you bring a form of ID with you to your initial consultation.

For your initial consultation we recommend you arrive about 5 minutes before your appointment time.

As we are a medical clinic, we require all patients to complete a detailed medical history form at their first appointment, this is necessary for the consultation and treatment planning process.

If there is a change in medical history, it is the responsibility of the patient to inform the Doctor.

Please note that Perfect Eyes Ltd is the medical arm of Dr Shah Desai’s practice. Perfect Skin Studio is the aesthetic arm of Dr Shah Desai’s practice & treatments will incur a VAT charge.

Please note that your personal data will be shared between both companies and all information will be treated as confidential and protected in accordance with Data Protection legislation.

Perfect Eyes Ltd New Patient Appointments

  • A comprehensive new patient consultation with Dr Shah Desai is £250.
  • A second opinion or revision consultation with Dr Shah Desai is £300.
  • A follow up consultation with Dr Shah Desai is £150.

Consultations are not redeemable against treatment costs.

This does not guarantee that Dr Shah Desai will take over your care.

For revision consultations, all documents relating to previous treatments/surgery will be needed prior to the consultation.

Perfect Skin Studio New Patient Appointments

A comprehensive new patient consultation with an associate doctor is £100. A follow up or review consultation is £50.

Consultations are redeemable against treatment costs.

For revision consultations, all documents relating to previous treatments/surgery will be needed prior to the consultation.

These can be sent to [email protected] before the date of your consultation.

We take full payment of your consultation fee to mitigate missed appointments and the administration costs we incur.

Please provide a minimum of 48 hours notice (2 working days, Mon-Fri) if you need to cancel or reschedule your appointment, so that we may make best use of our appointment diary. The fee is non-refundable if you do not attend without adequate notice.

Please do NOT attend the clinic for an appointment if you are unwell or are undergoing acute dental treatment. If you are unsure, please call and discuss with Dr Shah-Desai’s team. Many treatments are contraindicated if you are unwell; this includes active dental & sinus infections, fever, colds, cold sores & local skin infections.

If you develop an illness prior to the appointment, which requires you to cancel at short notice, please present a valid doctor’s certificate for a refund of any deposit paid.

Please arrive makeup free for your appointment to allow for clinical photography.

You are required to attend appointments alone, unless you require the assistance of a chaperone. Please contact us to discuss this prior to the appointment.

The building has disability facilities available (access ramp / toilet).

Should you require the assistance of an interpreter, please let us know in advance so that we can book this service for you.

Routine review appointments are offered after treatment with Toxin as a courtesy. No additional treatment or ‘top up’ is provided free of charge for fillers. Free top up for toxin is only offered within a review period of 2-3 weeks post treatment.

New patients are seen for consultation and assessment; except in exceptional circumstances, with prior arrangement, we do not offer treatment on the first visit.

Please bring a form of ID to your initial consultation appointment.


Please do not bring accompanying children to the clinic, as due to Health Care Commission regulations we are unable to allow anyone under the age of 18 into the clinic building.


  • You will be advised of the full costs of any treatment plan proposed and agreed, including that of any maintenance treatment, before any treatment is undertaken.
  • Payment is taken, in full at the time of treatment.
  • The clinic accepts all major debit and credit cards. We do not accept cash payment.
  • Finance options are available, please ask the team for further details about how to apply for this via PLIM limited.
  • A discretionary deposit will be taken for some treatments and in some circumstances.


Fees charged for treatment are for the delivery of a treatment and the accompanying service, which is inclusive of:

  • Consultation and assessment
  • Provision of information and advice
  • Safe treatment with evidence based products
  • Follow up appointments and aftercare advice and support as appropriate

Whilst we undertake to provide excellent service; factual, honest and ethical advice, safe, expert treatment in experienced hands and only the best products, we cannot guarantee your results and cannot offer refunds if the results achieved fail to meet your expectations, or you change your mind after commencing treatments of a “package price” treatment.

Skin care products are non-returnable/refundable.
